Où en est-on de la lutte contre le cancer aujourd’hui en France ?
Pr Axel Kahn : En 2018, il y a eu 382 000 nouveaux cas de cancers, 200 000 concernant les hommes et 182 000 chez les femmes. Aujourd’hui, le taux de guérison définitif est supérieure à 50%.

Il y a des cancers que l’on arrive à traiter presque tout le temps comme le cancer du testicule, la maladie de Hodgkin, par exemple, mais certains restes redoutables comme les glioblastomes qui sont des tumeurs cérébrales. Le cancer du pancréas et le cancer du poumon restent grave aussi.
De plus en plus de personnes guérissent alors que l’incidence du cancer augmente pourquoi ?
L’augmentation du nombre de cancers est liée à plusieurs événements : la progression de la longévité de vie entre autres car le cancer est une maladie de la deuxième partie de la vie et certains facteurs comportementales aussi. Il y a, par exemple, une augmentation de l’incidence du cancer du poumon chez les femmes égale à 5,3% et c’est très conséquent. L’an prochain, il tuera plus de femmes que le cancer du sein. Cette incidence est principalement due à l’évolution des comportements tabagiques de ces dernières par le passé et dont on constate les retombées aujourd’hui. Le retard des premières grossesses serait également impliqué puisqu’aujourd’hui elle intervient en moyenne vers l’âge de 30 ans, or on sait que la grossesse a un effet protecteur contre le cancer.
Dans d’autres cas, il faut reconnaître qu’aujourd’hui, certains cancers augmentent et on ne sait pas pourquoi, mais les recherches se poursuivent.

Quels sont les axes prévus pour améliorer la prévention ?
La priorité est la lutte contre les 3 causes fondamentales de cancers. Il faut continuer la lutte contre le tabac (inventés il y a 12 ans par la ligue) et favoriser la multiplication des espaces qui l’interdise. Concernant l’alimentation, nous sommes très engagés contre l’utilisation des nitrites dans la maturation de la viande. Et enfin, il y la lutte contre l’ alcoolisme. C’est LE trou noir du pays à cause du poids économique et culturelle qui pèse en France. On l’a vu avec le recul de Santé publique France sur l’organisation du Dry January (janvier sans alcool) mais que nous avons continué d’organiser de notre côté. On le voit aussi avec le refus de taxer les bières ultra fortes pourtant nous sommes inquiets des nouvelles pratiques de consommation d’alcool chez les jeunes car le binge drinking est extrêmement cancérigène.

Concernant les traitements où en est-on des avances ?
Les grands espoirs thérapeutiques aujourd’hui ce sont les immunothérapies. Elles donnent des résultats dans les formes de cancers du sein les plus graves et du cancer du poumons dans une minorité de cas, mais il y a une vraie percée dans ce type de traitement. Pour ce qui est des cancers dont ignore encore beaucoup de choses toutes les méthodes d’analyses doivent être mobilisées, mais je pense qu’on ne s’en sortira pas sans intelligence artificielle et pour ça nous disposons d’équipe Françaises extraordinaires et puissantes heureusement.
